Health Screening Form
Everyone entering the building must fill out the short COVOD-19 Health Screening Form. This includes all teachers, parents, and students.
You can fill out the form in one of three ways:

On the day of your lesson, click the "Online Form" button below. Simply fill out the form and click submit.

Or you can click the "Download Form" button. Then print out the form, fill it out, and bring it with you for your lesson.

If you do neither, you will have to fill out the form in person when you arrive. We would prefer to do this as few times as possible, so please use one of the other options if you can.
COVID-19 Information
After careful consideration, we have decided to reopen in a limited capacity. We will be following strict health and safety guidelines, based on procedures put in place by similar organizations and input from the Hamilton County Health Department.
Although much care and thought has been put into these plans, the situation is ever-changing. Following our reopening, we may make changes to our procedures based on new information or updates to county, state, and federal guidelines.
Requirements for All Students and Teachers

Before you arrive

Do not come to the MACC if you have been sick or had a fever in the last 24 hours.

Only students and teachers may enter the building unless permitted by the MACC staff.

No children other than the child taking the lesson may enter the MACC.

All water fountains in the building will be turned off, so bring any drinking water you might need.
When you arrive

All persons entering the building must wear a mask!

Students shall wait in their car until they receive a call or text from their teacher to enter the building.

Upon entering the building, each person shall use the provided hand sanitizer in the lobby.

Each person shall have their temperature taken with a no touch thermometer. Anyone with a temperature of 100.5 degrees or higher will not be allowed to enter.

Every person entering the building must complete and sign a brief health statement.
During your class

The teacher will meet the student at the front entrance and escort them to and from the classroom.

A mask shall be worn at all times in the classroom unless social distancing of at least 10 feet can be achieved or the lesson being given requires the mask to be removed. If a mask is removed for any reason, social distancing of 10 feet is required.
After you leave

If you discover you have been exposed to COVID-19, you must immediately notify the MACC!

We would also appreciate being informed if you have developed multiple COVID-19 symptoms, so that we can take necessary precautions.

Consult your physician if you suspect you may have COVID-19, and inform the MACC of their recommendations.
What we're doing

All doors to classrooms will be left open at all times.

Hand sanitizer will be provided in each classroom.

Items such as doorknobs and light switches that are commonly touched will be cleaned throughout the day.

Teachers shall be responsible for cleaning any items and instruments used by multiple students during the class.
In the Event of an Infection
In the event that a teacher or student has been verified to be infected by the COVID-19 virus, they will not be allowed back in the MACC for a minimum of 14 days and until released from isolation by the County Health Department. Anyone who had direct contact with that student or teacher will not be allowed back in the building for a minimum of 14 days and until released from isolation by the County Health Department.
All of us at the MACC would like to ask for your help in protecting the health and safety of everyone who uses the MACC. We can only remain open if everyone does their best to limit the spread of the virus by following our guidelines. The last thing we want is for anyone in our MACC family to be hurt in any way, so we are incredibly thankful for your cooperation.
